1. Cardiac Catheterization Room Charges.

1. Charges for inpatient stays in excess of 3
nights. Additional charges for these days including room and other
charges such as Pharmacy will be added to the package price at a special
discount of 20% to normal hospital rates. |

2. Accommodations (for one day in the Coronary Care
Unit (CCU) and 3 days stay in single room at Enclave-Ruby’s
lifestyle floor including : )

2. Charges for complex cases requiring emergency
surgery, balloon pump or other conditions requiring equipment and
hospital stays in excess of those specified. |

3. Laboratory Testing

3. Balloon catheters in excess of one (1). |

4. Radiology Studies

4. Outpatient fees and expenses incurred before
admission. |

5. Heart Centre

5. Non-invasive cardiology testing (except more than
one EKG & Echocardiography peripheral) including stress testing,

drinks, telephone charges and guest meals will be charged at the normal
