1. Cardiac Catheterization Room Charges.

1. Charges for inpatient stays in excess of 3
nights. Additional charges for these days including room and other
charges such as Pharmacy will be added to the package price at a special
discount of 20% to normal hospital rates (except on medicines). |

2. Accommodations (for one night in the Coronary
Care Unit (CCU) and two nights in single room at Enclave-Ruby’s
lifestyle floor including : )

2. Charges for complex cases requiring emergency
surgery, balloon pump or other conditions requiring equipment and
hospital stays in excess of those specified. |

3. Laboratory Testing

3. Outpatient fees and expenses incurred before
