The Package Includes	The Package Excludes
1. Operating Room Charges Operation Theatre charges Hemodynamic Monitoring Nursing and Technician room	1. Charges for inpatient stays in excess of 2 nights. Additional charges for these days including room and other charges will be added to the package
2. Accommodation (for 6 nights in The Enclave- Ruby's Lifestyle Floor) Room Charges In-patient Nursing Charges Service Charges Regular Menu Food Charges	2. Charge for other procedure if needed to be done.
3. Laboratory Testing Routine laboratory tests necessary for the procedure as ordered by the attending physician	3. Charges for intensive care, if necessary.
4. Pathology Studies Pathology Specimen	4. Charges for using more than 1 probe.
5. Radiology Studies Chest X-Ray, if medically indicated	5. Out-patient fees and expenses incurred before admission.
6. Heart Center EKG: for patient age above 40 years old or history of cardiac problems Echocardiography	6. Items of a personal nature such as in-room soft-drinks, telephone charges and guest meals will be charged at the normal rates in effect.
7. Medical Supplies Equipment and Medical supplies necessary for the procedure	7. Take home medications and supplies.

Patient Eligibility

The package is applicable for elective, uncomplicated patients. Patients with the following conditions are considered to be complex or high-risk cases and will not qualify for the package:

Diabetes out of control
Uncontrolled bleeding disorders
Metastasis of cancer to the urinary system
HIV infection
Organ failure/ Renal Insufficiency
Cardiac problem, if any, which may include Myocardial Infraction or post-CABG patient

Add on Charges : If the patient request a new, exclusive, single-use ADD probe, or if the patients condition requires single use ADD probe (e.g. prostates larger than 50 grams), an additional fee will be charged.
